Separate eligibility from reimbursement arithmetic
An invented bill contains a $180 consultation, $45 prescription, $60 monitoring test and $90 training session. If only the first three are eligible, the subtotal is $285, not the full $375. Apply the actual contract’s cost-sharing formula only after each line has been assessed. These figures are an accounting illustration, not treatment prices or a benefit determination.
